That would be “Bless their little ole hearts” down here in the South.
In Texas it’s just “Bless their hearts.” Out here you can say any insulting thing in the world and get away with it if you just add the magical phrase:
“That’s the ugliest baby I’ve seen in my whole life, bless her heart.”
“Her daddy is the biggest drunk West of the Pecos, bless his heart.”
“Stephen’s as crooked as a dog’s hind leg, bless his heart.”
…and so on.
